When you click the link and then click on "Request Sample" box you will be redirected to a page that says the sample request must be accessed through the Costco website. I think there is a link on that page that will take you to the Costco main page. If you type 'sample' in the search box, the diaper sample will come up. You can then click the link from there and will be directed to the form to complete for the free diaper.
