Purchase PediaSure Powder
Look for it in the baby/toddler aisle of your local retailer.

STEP 2
Get your Rebate Form
Visit pediasure.com/powder-rebate and follow the instructions. You’ll need your receipt
and the UPC code from your package of PediaSure Shake Mix. When you’re finished,
print the rebate form.

STEP 3
Mail in Your Rebate Form
Mail your completed form along with your original receipt with the purchase price circled, and the UPC Code to the address listed on the form. Your rebate request must be received by 1/31/2015. You should receive your rebate check in 6 to 8 weeks.
